    The patient checks in at the front desk when they arrive.


    The day before a patient's appointment, the front desk staff prints a shadow chart. Any outside documents, such as lab reports, were scanned into the system as they are received. Included in the shadow chart are the last progress note, all lab reports received since the last visit, the demographic sheet, and a fee slip.

    When the patient arrives for her appointment, they are given the printed demographic sheet and medical history form and asked to review them. If there are any changes the patient will note them on the form and then the receptionist will re-scan the changed form back into the system.

    The receptionist then attaches the shadow chart to a clip board. The clip board is then handed to the nurse to take with her when she escorts the patient to the exam room.

    Tagging the documents can occur immediately or at a later time when the receptionist has the time to complete the task. The images are tagged with:
      Date of service
      Type of document (progress note, driver's license, insurance card, etc.)
      Patient name and patient number
    Once the document is tagged they are immediately available on the network for other staff members to access.

    It should be noted that a paper patient file folder is never created. This eliminates the cost of a paper file and the time it takes to create a paper patient file. With supplies at $3.50 per file, this adds up to a significant savings annually.
